Developed by St. Louis Children's Hospital and is used to gather information before putting together a customized Health Information Packet for patrons in the Family Resource Center.

 •

Subject Codes and Headings   Posted 7/27/07
Developed at St. Louis Children's Hospital, modeled after the Emily Center at Phoenix Children's Hospital and is used to categorizes books, brochures and information.  The two letter code is used on books and videos in addition to the National Library of Medicine Classification System.  Brochures and other information are filed according to the two letter subject code.  Information requests are also categorized according to these codes.

 •

Activity Log  Posted 7/27/07
Developed at St. Louis Children's Hospital and was used to track patrons on a daily basis.  The data was then entered into the computer.  This template was used to create an access database for data entry, directly into the computer, eliminating the need to manually count the patrons and their activities.

 •

In House Request for Information Fax Form Posted 7/27/07
Developed by St. Louis Children's Hospital and is used by the staff on the patient care units, especially the charge nurses, when making rounds.  He or she writes down the new patients and diagnosis, families who might have questions, etc. and faxes the form to the Family Resource Center.  The staff in the Family Resource Center then creates a customized Health Information Packet and delivers it to the patient care unit for the patient and family.
